In Canada, three biostimulators are now authorized by Health Canada: HArmonyCa, Radiesse, and Sculptra. All three share the same broad goal. How they achieve it, and which one suits a given patient, depends on composition, mechanism, treatment timeline, and individual anatomy.

What Is a Collagen Biostimulator?

A collagen biostimulator is an injectable treatment that addresses this by stimulating fibroblasts, the cells responsible for synthesizing collagen and elastin, to produce new structural tissue. Rather than replacing lost volume with a foreign substance, biostimulators prompt the skin to rebuild its own framework.

According to global consumer research conducted by Allergan Aesthetics involving 12,360 respondents, 96% reported a concern about an area of their face, with 40% concerned about skin quality and 39% about loss of skin tightness. When asked whether they would consider a non-surgical treatment that stimulates the skin’s own collagen production, two-thirds said they would (Allergan Consumer Beauty Insights Study 2021, via Allergan Aesthetics Canada).

HArmonyCa, Radiesse, and Sculptra each approach collagen stimulation through different active ingredients and mechanisms.

HArmonyCa: The Hybrid Approach

HArmonyCa is a dual-action injectable. It received Health Canada authorization in May 2026, making it the first hybrid HA + CaHA injectable authorized in Canada (Allergan Aesthetics Canada).

It is composed of two active ingredients combined in a single syringe using Allergan’s patented DART (Dual Action Rejuvenating Technology):

  • Hyaluronic acid (HA): Provides immediate volumization and structural support
  • Calcium hydroxyapatite (CaHA) microspheres: Stimulate fibroblast activity and progressive collagen production over time

The HA component acts immediately after injection, adding volume and creating a visible lifting effect in the treated area. It also functions as a carrier gel, distributing the CaHA microspheres evenly within the deeper tissue layers so that biostimulation is consistent across the treatment zone.

Over the following weeks and months, the CaHA microspheres activate fibroblasts to produce new collagen and elastin. A clinical study using ultrasound and elastography confirmed increases in skin density, viscoelasticity, and firmness beginning at 60 days post-treatment, with collagen production confirmed at 180 days. Facial tissue volume increased by a median of 2.2 mm at 180 days, as measured by Vectra H2 3D imaging (Urdiales-Gálvez et al., Journal of Cosmetic Dermatology, 2023).

In a post-marketing clinical study of 162 patients, 91.6% showed measurable improvement in the general appearance of treated facial areas. Patient satisfaction was rated 4.1 out of 5 at up to 19 months post-treatment (Allergan Aesthetics Canada).

Radiesse: Calcium Hydroxyapatite Biostimulation

Radiesse is a Health Canada-authorized injectable composed of calcium hydroxyapatite (CaHA) microspheres suspended in a carboxymethylcellulose (CMC) gel. CaHA is a biocompatible mineral compound with a long history in aesthetic medicine.

When injected, Radiesse provides immediate structural volumization through the physical presence of the CaHA-CMC gel. Simultaneously, the CaHA microspheres function as a cellular scaffold. The surrounding tissue responds by producing collagens I and III, elastin, proteoglycans, and new vasculature.

A comprehensive narrative review published in the Aesthetic Surgery Journal describes this mechanism: CaHA drives soft tissue regeneration through a cell-biomaterial interaction, with minimal adaptive immune cell recruitment and without chronic inflammation, distinguishing it from some other biostimulatory materials (Aguilera et al., Aesthetic Surgery Journal, 2023).

Radiesse does not contain hyaluronic acid. The immediate effect is structural, produced by the volume of the CaHA-CMC gel itself rather than the hydrating, softening effect of HA.

It is a well-established treatment with an extensive clinical evidence base for patients and practitioners familiar with biostimulators.

Sculptra: Poly-L-Lactic Acid Collagen Rebuild

Sculptra (Galderma) is a Health Canada-authorized injectable poly-L-lactic acid (PLLA) biostimulator. PLLA is a biodegradable synthetic polymer that has been used in medical applications, including dissolvable sutures, for decades (Cleveland Clinic).

Unlike HArmonyCa and Radiesse, Sculptra provides no immediate volumizing effect. PLLA microparticles are injected into the deeper layers of the skin, where they trigger a gradual inflammatory response that stimulates fibroblast proliferation and new collagen synthesis. Because this cellular cascade develops over time, the visible result emerges progressively across multiple treatment sessions spaced four to six weeks apart (Narins et al., PMC, 2012).

Sculptra gradually improves overall skin quality by stimulating your body’s natural collagen production. As collagen is restored, the skin becomes firmer, thicker, smoother, and more elastic, with visible improvements in texture, fine lines, radiance, and overall skin quality. Some patients also notice a subtle improvement in the appearance of enlarged pores as the skin becomes more structurally supported.
